The Crisis and Resilience Fund (CRF) Hospital Agent plays a vital role in supporting people who are facing financial hardship, housing insecurity, social isolation, or other barriers that may impact their ability to leave hospital safely and remain well within their community.

Working as an integrated member of the hospital discharge system, the postholder will identify and support individuals whose nonclinical needs place them at risk of delayed discharge, readmission, or deteriorating wellbeing. The role bridges hospital services and community support, ensuring people can access practical help, wellbeing support, financial assistance, grants, community resources, and longer-term resilience-building opportunities.

You will work closely with hospital teams, Adult Social Care, voluntary sector partners, housing providers, and community organisations to remove barriers to discharge and improve outcomes for some of Somerset’s most vulnerable residents. You will not only deliver direct support to individuals but will work closely with the wider Crisis & Resilience Fund team to develop and embed the service within hospital pathways.

As a key link between the hospital and community settings, they will contribute to service innovation, partnership development and the identification of emerging needs and opportunities that improve both discharge outcomes and long-term community resilience.
